6.06 Nephrotic syndrome, with A and B
A. Laboratory findings as described in 1 or 2, documented on at least two occasions at least 90 days apart during a consecutive 12-month period:
- Proteinuria of 10.0 g or greater per 24 hours; or
- Serum albumin of 3.0 g/dL or less, and
- Proteinuria of 3.5 g or greater per 24 hours; or*
- Urine total-protein-to-creatinine ratio of 3.5 or greater.*
AND
B. Anasarca (see 6.00C6) persisting for at least 90 days despite prescribed treatment.
What you must show
- Persistent heavy proteinuria documented twice, 90+ days apart, by either:
- ≥ 10 g per 24 hours, or
- Serum albumin ≤ 3.0 g/dL and either ≥ 3.5 g protein per 24 hours or a urine protein-to-creatinine ratio ≥ 3.5; and
- Anasarca lasting at least 90 days despite treatment.
Proof tips
- Use 24-hour urine collections or spot urine protein-to-creatinine ratios; make sure they are clearly dated and two results are at least 90 days apart.
- Include serum albumin values aligning with the dates above.
- Show persistent edema: repeated exams documenting anasarca, escalating diuretics, salt restriction, and nephrology notes confirming refractoriness.
- Include biopsy reports (if done) establishing the underlying glomerulopathy (e.g., FSGS, membranous). While not required, it adds weight.
